Best HSA/FSA Products That Cost More Than $500

The saving on pre-tax money grows with the price tag, so the big purchases you have been putting off are where an HSA or FSA does the most. Here are seven worth it, which need an LMN and which don't, and how the money works when the bill is large.

The quick answer

What are the best HSA/FSA eligible products over $500?

A therapeutic mattress leads, with six more from strength training to a home sauna.

Some need no paperwork, like compression boots or a hearing-aid accessory; others need a Letter of Medical Necessity (LMN) tying them to a diagnosis. Each card says which.

Estimated pre-tax savings: about $174 to $2,307 per item, at a 30% combined tax rate.

Caeli handles the LMN at checkout: a telehealth intake in under 3 minutes.

Big-ticket buys

The bigger the purchase, the more pre-tax money saves

The products that change how you sleep, move and breathe tend to be the expensive ones: a mattress built for a bad back, a strength trainer that sets the weight for you, compression boots, an air purifier built to run quietly. Paying with pre-tax dollars takes roughly 30% off in effect, so the saving scales with the price: on the Saatva Rx mattress below, the saving is about $1,050.

Two rules make big purchases easier than people expect. A Health FSA makes your whole annual election available from the start of the plan year: IRS Publication 969 says you "must be able to receive the maximum amount of reimbursement… at any time during the coverage period, regardless of the amount you have actually contributed." And HSA money has no deadline; the same publication says "You don't have to make withdrawals from your HSA each year," so you can pay now and reimburse yourself later. One catch: if you contribute to an HSA, your FSA can only be a limited-purpose one (dental and vision), which will not pay for anything here.

Some of these products need no paperwork. Publication 502 names hearing aids outright, with the "batteries, repairs, and maintenance needed to operate" them; the hearing-aid mic, the compression boots and the red light panel below are HSA/FSA eligible without an LMN too. Others are things anyone might buy, so they count when you use them primarily for a diagnosed condition, and an LMN is the proof.

How an LMN works

Usually an LMN means a doctor's appointment, often a co-pay, and a request for the letter. With Caeli it happens at checkout: a telehealth intake in under 3 minutes, and a licensed clinician reviews and signs, usually in minutes. The LMN is dated before you buy, names your condition, and covers the product and similar ones for 12 months. The conditions below are common examples across these products, not the full list.

Conditions a clinician can cite include:

  • Low back pain
  • Sciatica
  • Osteoarthritis
  • Asthma
  • Allergic rhinitis
  • Obesity

You don't have to keep track of which is which. On the product you are looking at, Caeli shows whether your benefits cover it and handles the LMN when one is needed.

One diagnosis, several purchases

When one condition covers more than one big buy

Low back pain is on the list an LMN can cite for the mattress, the desk, the strength trainer and a massage chair alike, so one diagnosis can support more than one of these purchases. Two more for the same reader: a zero-gravity massage chair, which needs an LMN, and a red light panel, which does not; the red light guide covers how people use it for joint pain. If a body-composition scale is on your list too, see the smart scale guide.

Human Touch Gravis ZG Chair

The top of the range: a zero-gravity massage chair

A massage chair that reclines to zero gravity, with air massage, heat and powered head, back and foot rests, controlled from an app or remote. 5-year limited warranty.

Before you claim

Two details for the paperwork

The mattress, desk, trainer, purifier and sauna are dual-use items. Read strictly, Publication 502 counts only "the excess of the cost of the item in a special form over the cost of the item in normal form"; in practice most administrators process the full price against a valid LMN, and your plan administrator can tell you how they treat it. The bigger the purchase, the more this matters, so ask before you buy if you are unsure.

Keep the LMN, dated before you buy, and the itemized receipt together; that is what a claim or an audit asks for, and with an HSA the records are yours to keep. Buy through Caeli and that part is done for you: every Buy-with-Benefits purchase is vaulted in the Reimbursement Vault, itemized and audit-ready.

Questions

Frequently asked

Can I buy a $2,500 item with my FSA before I've contributed that much?

Yes, up to your annual election. IRS Publication 969 requires a Health FSA to make the full amount you elected available at any time during the coverage period, regardless of how much you have contributed so far.

If I have an HSA, can my FSA pay for these?

No. If you contribute to an HSA, your FSA can only be a limited-purpose one, covering dental and vision. Pay with your HSA instead; it has no deadline.

Why does a hearing-aid accessory need no LMN when a mattress does?

Publication 502 names hearing aids, and what is needed to operate them, as medical care outright. A mattress is something anyone might buy, so it counts only when it is used primarily for a diagnosed condition, and the LMN is the proof.

Can I pay now and reimburse myself from my HSA later?

Yes, for qualified expenses incurred after you opened the HSA; there is no deadline for the withdrawal. Keep the receipt, and the LMN where one is needed, with your records.

Is the Tonal membership covered too?

The membership is a separate monthly cost from the trainer. Whether it qualifies depends on the same medical-purpose test and on your plan, so check with your plan administrator before you claim it.

The big purchase, finally.

The mattress you have been meaning to replace, the trainer you keep looking at: at roughly 30% off in effect, the big purchase is where your benefits go furthest. Browse the shop and let Caeli handle the LMN where one is needed.
