What to Buy With an FSA When You Have $1,000 Left

A $1,000 balance is more than any plan can carry over for 2026, and it is enough for the kind of health gear most people never buy after tax. Here is what your card covers outright, the much wider shelf an LMN opens if you have a health condition, and how much of the balance is on the clock.

The quick answer

What should you buy with $1,000 left in your HSA or FSA?

Premium gear your card covers today, and a far wider shelf with an LMN.

Compression boots and an FDA-cleared acne mask need no paperwork. With an LMN for a condition you have, an air purifier or a rower can fit too. $1,000 is above the 2026 carryover cap, so check your deadline.

Estimated pre-tax savings: about $17 to $297 per item, at a 30% combined tax rate.

Caeli handles the LMN at checkout: a telehealth intake in under 3 minutes.

Bucket one · No LMN needed

Covered on your card, no paperwork

Good news first: $1,000 is enough for the health gear people admire and rarely buy with after-tax money. Wireless compression boots from Therabody, an FDA-cleared LED mask from Omnilux, a handheld check-up device from Withings. Every item below fits $1,000 on its own, and none needs an LMN: you pay with your HSA or FSA card. Two more things follow the picks: the much bigger shelf a Letter of Medical Necessity (LMN) opens if you have a health condition, and why the clock matters at this balance.

FDA-cleared in 2025, BeamO puts a 1-lead ECG you record in 30 seconds, a digital stethoscope and a contactless thermometer in one handheld device. Why no paperwork: the IRS counts payments for "the diagnosis, cure, mitigation, treatment, or prevention of disease" as medical care, and Publication 502 includes devices used to diagnose and treat illness. A heart check, a TENS unit and an FDA-cleared acne treatment sit squarely on that side of the line. One way to use nearly all of it: the boots and the acne mask together come in just under $1,000 at today's shop prices.

Bucket two · With an LMN

Have a health condition? The shelf gets much wider

This is the part most people miss. Plenty of premium products are dual-use: a chair, an air purifier, a rowing machine. The IRS counts them as medical care when you buy them to treat a diagnosed condition, and an LMN is how you show it: a clinician names your condition and ties the product to it. Many people already have a condition that qualifies, such as asthma, low back pain, high blood pressure or insomnia, without knowing it opens this shelf. Each product below also fits $1,000 on its own.

Sleep and breathing. A Chilipad cools the bed with water rather than blowing air, and a medical-grade purifier cleans the air in the same room. Our bed cooling guide compares Chilipad and BedJet systems side by side, and our air purifier picks line up eight HEPA machines, from a Levoit under $100 to this Austin Air.

Sleepme Chilipad Cube

Sleepme Chilipad Cube

Sleep that runs hot, with a diagnosis

The simplest Chilipad: water runs through a pad on the mattress at one temperature all night, about 10°F below the room, set from a physical remote. It needs no Wi-Fi, no app and no account, and there are no wires in the pad.

The desk. If your back is the reason you are reading this, a better chair and a way to walk while you work are two changes worth raising with a clinician. Our office chair guide goes up to Herman Miller's Embody Gaming Chair, our lower back guide covers desks and lumbar support, and our walking pad guide explains when a pad needs an LMN and when an LSA is the easier route.

Branch Ergonomic Chair Pro

Branch Ergonomic Chair Pro

Heart and metabolism. A scale that takes an ECG and an omega-3 a practitioner recommends both belong here. Publication 502 counts a supplement only when it is "recommended by a medical practitioner as treatment for a specific medical condition diagnosed by a physician", and the LMN is that recommendation on paper. Our smart scale guide compares all five Withings scales, and our prediabetes guide builds a kit around a blood sugar diagnosis.

Withings BodyScan 2 Black

Withings BodyScan 2 Black

Heart and body composition in one morning scan

Withings' 2026 flagship. A retractable handle with a color display adds a 6-lead ECG through FDA-cleared software, fat and muscle across six body zones, Vascular Age and blood oxygen (SpO2). The battery is rechargeable, rated up to a year per charge.

Movement and heat. For high blood pressure, low back pain or fibromyalgia, a clinician may recommend exercise or heat as part of your care. A rower gives you a low-impact workout for the whole body, and an infrared sauna blanket brings the heat home. Our arthritis guide pairs heat with TENS and cold therapy.

Concept2 RowErg

Wearables sit in between. A smart ring, a running watch or an over-the-counter glucose sensor counts when you buy it for a diagnosed condition or on a clinician's recommendation; the IRS says "a generalized desire to monitor or improve health is not sufficient" (Q15). Keep that record, and an LMN is the cleanest proof. Our smart ring guide and Garmin guide walk through the models.

How an LMN works

Usually an LMN means a doctor's appointment, often a co-pay, and a request for the letter. With Caeli it happens at checkout: a telehealth intake in under 3 minutes, and a licensed clinician reviews and signs, usually in minutes. The LMN is dated before you buy, names your condition, and covers the product and similar ones for 12 months. The conditions below are common examples across these products, not the full list.

Conditions a clinician can cite include:

  • Insomnia
  • Obstructive sleep apnea
  • Asthma
  • Allergic rhinitis
  • Low back pain
  • Fibromyalgia
  • High blood pressure
  • Obesity
  • Type 2 diabetes
  • Hyperlipidemia

These eight are a start. Caeli's Picks has many more editorial guides for different health and lifestyle needs, from sleep and skin to the home office, each written around the products that condition unlocks.

You don't have to work out which of these needs an LMN. On the product you're looking at, Caeli shows whether your benefits cover it, and it handles the LMN when one is needed.

The clock

Why $1,000 can't just wait

A Health FSA is "generally 'use-it-or-lose-it'", and IRS Publication 969 lets a plan soften that with a carryover or a grace period, never both. The carryover is capped: for 2026 the IRS maximum is $680 (Rev. Proc. 2025-32). So on a carryover plan that uses the full cap, at least $320 of a $1,000 balance is lost if you don't spend it, and more if your plan's cap is lower. A grace period gives you up to 2½ months to spend all of it, to March 15 on a calendar-year plan. With neither, the whole balance goes at the end of the plan year. A run-out period is only extra time to file claims.

Not sure which rule your plan uses? Your plan documents say, and either list above gives you somewhere good to put the money before it goes.

Make it count

Stretch it further

Read strictly, Publication 502 counts only the extra cost of a dual-use item over its ordinary version; most administrators process the full price against a valid LMN. Either way, keep the LMN with the itemized receipt. Buy through Caeli and that part is done for you: every Buy-with-Benefits purchase is vaulted in the Reimbursement Vault, itemized and audit-ready.

Questions

Frequently asked

How much of a $1,000 FSA balance could I lose?

On a carryover plan with the full 2026 cap of $680, at least $320 is forfeited if unspent; more if your plan's cap is lower. With a grace period you can spend all of it in the extra time; with neither, all of it is at stake.

Do I need an LMN to spend $1,000 of FSA money?

No. Everything in the first list goes on your HSA or FSA card with no paperwork. The second list, dual-use products such as an air purifier or a rower, needs an LMN that ties the product to a diagnosed condition.

Which conditions can an LMN cite?

It depends on the product. Asthma and allergic rhinitis are common for air purifiers; low back pain for chairs, rowers and saunas; high blood pressure, obesity and type 2 diabetes for scales and treadmills. Each product category has its own, longer list; these are common examples.

Can an LMN be dated after I buy?

No. The LMN has to be dated before the purchase to back it up. With Caeli the intake takes under 3 minutes and a clinician usually signs in minutes, so you can get it at checkout.

Are compression boots FSA eligible?

Therabody sells JetBoots Prime as "HSA/FSA Accepted", and you can buy them with your HSA or FSA card without an LMN. Plans treat recovery devices differently, so keep the receipt in case yours asks what they are for.

Is a smart ring or fitness watch FSA eligible?

When you buy it for a diagnosed condition or on a clinician's recommendation, yes; a general wish to track your health is not enough, per the IRS. Keep that record, and an LMN is the cleanest proof.

Legs up, on money that was about to go.

A long walk, then an hour in the boots with your feet up, or a cooler night's sleep on a Chilipad: either is a better ending for $1,000 than a forfeited balance. Browse everything FSA eligible in the Caeli shop, or the products that need an LMN.
