Two routes: an LMN for your HSA or FSA, or your employer's LSA
HSA or FSA. The IRS draws the line at the reason you walk. Exercise "for the improvement of general health" is not medical care, even when a doctor recommends it, but the IRS says a gym membership bought for the sole purpose of treating "a specific disease diagnosed by a physician (such as obesity, hypertension, or heart disease)" is (IRS FAQ). A walking pad follows the same logic under IRS Publication 502 and IRC §213(d): with a diagnosis and an LMN tying the pad to it, it is medical care. A "walk more" note from a checkup is not an LMN.
The conditions a clinician can cite for a treadmill through Caeli include obesity, type 2 diabetes, high blood pressure, high cholesterol, metabolic syndrome, low back pain and knee or hip osteoarthritis. Whether yours covers a walking pad is set by your plan, so check it before you count on it.

Publication 502's strict reading counts only "the excess of the cost of the item in a special form over the cost of the item in normal form"; in practice most administrators process the full price against a valid LMN, and your plan administrator can say how they treat it. An LSA is employer money, not pre-tax income, so there is no tax saving to quote for it.

Is a walking pad HSA eligible without an LMN?
No. A walking pad bought for general fitness is not a medical expense, even if a doctor told you to walk more. With an LMN tying it to a diagnosed condition such as obesity, high blood pressure or low back pain, it is.
Can I use my LSA for an under-desk treadmill?
Often, if your employer's plan has a fitness or home-office category that includes it. There is no LMN involved, and your employer, not the IRS, decides what the LSA covers, so check the plan's list.
Should I use my LSA or my HSA for a walking pad?
