The new IRS rule

Smart rings have become the easiest health tracker to live with. A few grams of titanium or steel on your finger reads heart rate, heart rate variability and blood oxygen through the night, and the newest ones run a week or two on a charge. RingConn and Samsung sell theirs with no subscription at all, and RingConn's Gen 3 includes sleep apnea pattern insights. Prices run from about $200 to over $400, which is exactly where pre-tax money makes a difference.
So is one HSA eligible? The IRS answered in September 2026, in a new FAQ on wearables (Q15). A ring is not a medical expense "merely because the device tracks information that relates to health generally". It is one when you buy and use it "primarily to assist in the diagnosis, cure, mitigation, treatment, or prevention of a specific disease". The facts that count include "a recommendation from a medical practitioner; the individual's symptoms, diagnosis, abnormal test results, or risk factors". What doesn't: "A generalized desire to monitor or improve health is not sufficient."
Q15 does not ask for a Letter of Medical Necessity (LMN). What it asks is that the reason be real: high blood pressure, an arrhythmia, suspected sleep apnea, insomnia, or a clinician who told you to track something. Here is where "subscription-free" matters. Q15 applies the same test to "accessories, add-ons, or software subscriptions": they count only if bought primarily for that medical purpose. A ring with no membership leaves one less thing to justify, every month, for as long as you wear it.

How an LMN works
The usual route to an LMN is a doctor's appointment, often with a co-pay. Do I need an LMN for a smart ring?

Can my HSA or FSA pay for an Oura membership?

Does a ring's sleep apnea alert count as a diagnosis?
No. RingConn says its rings are not intended to diagnose disease. Treat an alert as a reason to see a clinician; if they diagnose sleep apnea, that diagnosis is what makes the ring, and more, a medical expense.
I have no diagnosis yet, just symptoms. Does that count?
It can. Q15 lists symptoms, abnormal test results and risk factors among the facts that count, alongside a clinician's recommendation. What does not count is a general wish to track your health.
