Many of the newer creams pair a skin-protectant medicine such as colloidal oatmeal with ceramides, the lipids that help hold the skin barrier together, and brands like Hypothesis and Skinfix make steroid-free formulas gentle enough for the face. So, can your HSA or FSA pay for them? Usually, yes. The IRS says over-the-counter drugs "may be paid or reimbursed by an HSA, FSA, Archer MSA, or HRA", prescription or not, and Publication 969 counts them as medical care for an FSA. The one nuance: Publication 502 excludes items "ordinarily used for personal, living, or family purposes" unless they are used primarily to treat an illness, and an everyday moisturizer is that kind of item.

The eczema products that qualify list a skin protectant or an anti-itch drug there: colloidal oatmeal, petrolatum or dimethicone, or 1% hydrocortisone. The creams are the no-paperwork part of an eczema routine. Beyond them is a second tier: products that are not medicines on their own but become HSA/FSA eligible with a Letter of Medical Necessity (LMN) that ties them to a diagnosed condition.
