Top brain health supplements: how an LMN unlocks pre-tax savings.

Thorne and Ritual make some of the most carefully formulated supplements you can buy, and you take them every month. With an LMN for a diagnosed condition, such as a low B12 level, depression or insomnia, your HSA or FSA can pay for them with pre-tax money. Here is what qualifies, five picks and the sleep support that goes with them.

The quick answer

Are brain health supplements HSA/FSA eligible?

Yes, with an LMN for a diagnosed condition, not for general brain health.

The IRS counts a supplement when a clinician recommends it to treat a diagnosed condition, such as a B12 deficiency or depression. A Letter of Medical Necessity (LMN) is how you show it.

Estimated pre-tax savings: about $6 to $28 per item, at a 30% combined tax rate.

Caeli handles the LMN at checkout: a telehealth intake in under 3 minutes.

The good news

Better supplements, paid for with pre-tax money

Supplements have grown up. Thorne puts its B vitamins in the active forms your body uses directly, such as methylcobalamin for B12, and makes a fish oil with 1,300 mg of EPA per serving. Ritual makes a vegan omega-3 from microalgae and publishes where each ingredient comes from. They cost more than the drugstore bottle, and you buy them every month, which is exactly where HSA and FSA dollars help. The key that unlocks them is a Letter of Medical Necessity (LMN).

Maybe a blood test came back low on B12, a clinician is treating you for depression, or you have not slept properly in months. Those are the moments a supplement belongs in a care plan. It helps to be clear about what the evidence says: NCCIH finds that a multivitamin or high doses of B vitamins have no clear benefit for cognition in well-nourished people. For a diagnosed condition the picture changes. The NIH Office of Dietary Supplements lists depression, confusion and poor memory among the symptoms of a B12 deficiency, which affects between 3% and 43% of older adults.

A supplement is not a shortcut to a sharper brain, but for a diagnosed condition it can be part of the plan, and your HSA or FSA can pay for it.

The rule, briefly

A diagnosis turns a supplement into medical care

IRS Publication 502 is direct about it: you can't count vitamins or supplements "unless they are recommended by a medical practitioner as treatment for a specific medical condition diagnosed by a physician." The IRS answers the HSA and FSA question the same way in its nutrition and wellness FAQ: yes, but only with that recommendation for a diagnosed condition. Supplements taken for general health are what the IRS calls "merely beneficial to general health." The "HSA/FSA eligible" badge on a brand's site is the store's label, not the rule.

You don't have to memorize any of this. On the supplement you are looking at, Caeli shows whether your benefits cover it, and when it needs an LMN, Caeli handles that too.

How an LMN works

Usually an LMN means an appointment with your doctor, often a co-pay, and a request for the letter. With Caeli it happens at checkout: a telehealth intake in under 3 minutes, and a licensed clinician reviews and signs, usually in minutes. The LMN is dated before you buy, names your condition and covers the product and similar ones for 12 months. The conditions below are common examples for these picks, not the full list. Memory loss on its own is not one of them; a clinician's workup is the right first step, and a B12 test is one of the simplest parts of it.

Conditions a clinician can cite include:

  • Vitamin B12 or other B-vitamin deficiency
  • Depression
  • Anxiety disorder
  • Insomnia
  • Magnesium deficiency

The picks

Five supplements with a clear job to do

Each pick matches one diagnosis a clinician can cite, from B vitamins for a deficiency to omega-3s alongside depression treatment. They run from the splurge, a high-EPA fish oil, to a budget B12 gummy.

Thorne Basic B Complex

Editor's pick for a B-vitamin deficiency

Thorne Basic B Complex

A diagnosed B12 or B-vitamin deficiency

Eight B vitamins plus choline in one capsule, in the active forms Thorne chose so the body can use them directly: 400 mcg of B12 as methylcobalamin, folate as L-5-MTHF and B6 as P5P. A bottle is 60 servings. The NIH notes that metformin may lower B12 absorption, one reason a doctor may test for it.

Thorne Super EPA Pro
#1

Thorne Super EPA Pro

The splurge: omega-3 alongside depression treatment

1,300 mg of EPA and 320 mg of DHA per two-gelcap serving, the most EPA of any pick. That matters because NCCIH says reviews suggest EPA may help more than DHA for depression, used alongside antidepressant treatment rather than in place of it. NCCIH is also clear that any benefit is uncertain and may be too small to matter, so this is a pick for your clinician's plan, not a treatment on its own.

Ritual Omega-3 DHA & EPA
#2

Ritual Omega-3 DHA & EPA

Mid-range: a vegan omega-3

500 mg of omega-3 from microalgae in one vegan capsule, with DHA and EPA in a 2:1 ratio and a citrus flavor Ritual designed for minimal burp-back. Every lot is third-party tested for heavy metals. It leans toward DHA, so if EPA is what your clinician wants, the Thorne above carries more.

Ritual Stress Relief
#3

Ritual Stress Relief

Special purpose: diagnosed insomnia or anxiety

One morning capsule with 80 mg of Shoden ashwagandha, 200 mg of L-theanine and 28 mg of affron saffron, NSF Contents Certified. NCCIH says some ashwagandha preparations may help insomnia and stress, while the evidence on anxiety is unclear. Ashwagandha isn't for pregnancy, breastfeeding, or thyroid or autoimmune disorders.

The same diagnosis

Sleep is part of brain health too

Insomnia shows up in the same care plans as stress and low mood, and it is a condition an LMN can cite for sleep supplements as well. NCCIH says melatonin may help jet lag and a delayed sleep-wake phase, while the evidence for long-term insomnia is thin, and it is not recommended for people with dementia. Magnesium belongs in the plan when a clinician finds you are low on it.

No need to work out which of these needs which paperwork. Caeli shows on each product as you shop whether your benefits cover it, and brings in the LMN only where one is needed.

The fine print

Tell your clinician, and keep the paperwork

Supplements are real substances, so bring the list to the clinician who writes the LMN. NCCIH notes omega-3s may extend bleeding time, which matters if you take a blood thinner, and that ashwagandha may be safe for up to 3 months, with no firm answer on longer use. If you have an HSA, any FSA you hold alongside it can only be limited-purpose (dental and vision), which won't pay for supplements (IRS Publication 969).

Keep the LMN and the itemized receipt together; that is what a claim or an audit asks for. When you buy through Caeli it is done for you: every Buy-with-Benefits purchase is vaulted in the Reimbursement Vault, itemized and audit-ready.

Questions

Frequently asked

Can I use my HSA for a supplement just to sharpen my memory?

No. The IRS excludes expenses that are "merely beneficial to general health", and NCCIH finds no clear cognitive benefit from vitamins in well-nourished people. If your memory has changed, see a clinician; a treatable cause such as a B12 deficiency is one thing they may check.

Thorne's site says HSA/FSA eligible. Do I still need an LMN?

Yes. The badge is the store's label, and the IRS rule for supplements asks for a clinician's recommendation for a diagnosed condition. The LMN is how you show that when your administrator asks.

Can I get an LMN for these supplements for memory loss or mild cognitive impairment?

Not through Caeli for these picks. The conditions our LMN intake accepts for them, depending on the product, include B-vitamin deficiency, depression, anxiety disorders and insomnia, not memory loss itself. Memory changes deserve a proper workup first.

Is omega-3 proven to treat depression?

Not on its own. NCCIH says the benefit is uncertain and may be small, and that omega-3s may work best alongside antidepressant treatment, with EPA possibly more useful than DHA. Take it as part of the plan your clinician sets.

Does one LMN cover my monthly refills?

The LMN covers the product and similar ones for 12 months, so the refills of that supplement through the year can rely on it. Keep each receipt with the letter.

Can I get the LMN after I buy?

Get it first. The LMN should be dated before the purchase to substantiate it, which is why Caeli issues it at checkout.

I have an HSA and an FSA. Which should pay?

If you contribute to an HSA, any FSA you also have can only be limited-purpose (dental and vision) and won't cover supplements, so use the HSA. With an HSA you keep the LMN and receipts yourself, because you substantiate your own spending (IRS Publication 969).

A steadier mind, on pre-tax money.

Here is to the supplements that actually earn their place: the B12 that fixes what a blood test found, the omega-3 that backs up your treatment, and a night's sleep that sets up the next day. If a clinician has diagnosed the reason you need them, your HSA or FSA can pay: browse the vitamins and supplements, and let Caeli handle the LMN.
